We're really proud to have contributed to the NHS London Carers and Hospital Discharge Toolkit. Focussed on improving the experiences of carers and the people they care for during the hospital discharge journey, the toolkit is packed with action-orientated top tips.

Designed for London hospitals and community providers, the toolkit has been co-produced in partnership with Carers UK, Carers First, Carers Trust and the London Association of Directors of Adult Social Services (ADASS).
